What is Respite Care?

Respite care provides family members or unpaid care givers of participants, an opportunity to pursue other commitments and activities. Respite care can also be provided in emergency situations where care givers will be away unexpectedly.

What is Transitional Care?

Transitional care provides participants with short-term specialised care and support after a hospital stay. Transitional care allows participants to gain their functional independence sooner, which could minimise the requirement for longer term support services.
